Here's the simple version: your medication (Micomp PB) already contains caffeine as one of its ingredients. Yerba mate brings even more caffeine to the table. Stack the two together and you're essentially doubling up on a stimulant, so you may notice things like a racing or pounding heart, jitteriness, trouble sleeping, an upset stomach, or feeling anxious and on edge.

This concern is based mostly on expert reasoning rather than hard studies, so it isn't an emergency. An occasional cup likely won't cause trouble for most people, but daily or large amounts of mate add up. Since the belladonna and ergotamine parts also affect your heart and circulation, it's worth running your mate habit by your pharmacist or doctor.

Here's the thing to keep in mind with this combination product. One of its ingredients, pentobarbital, is a barbiturate meant to calm you down and, at higher doses, help you feel drowsy or sleepy. Yerba mate is loaded with caffeine, which does the opposite, it wakes the brain up. So when you sip mate alongside this medicine, the stimulating caffeine can work against the calming, sleep-promoting side of the pentobarbital, possibly leaving you feeling jittery or less relaxed than expected.

To be fair, this is more of a theoretical tug-of-war than a proven danger, and remember the product itself already contains caffeine. Still, if you find the medicine isn't settling you the way it should, chat with your pharmacist or doctor before changing anything.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Theoretically, yerba mate might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that yerba mate extract inhibits CYP3A4 enzymes. Theoretically, taking yerba mate may increase levels and adverse effects of CYP3A4 substrates.

Here's the practical picture. This combination pill already contains caffeine, and yerba mate brings its own caffeine to the party. So the most likely thing you'd actually notice is too much stimulation: jitteriness, a racing or pounding heart, trouble sleeping, or feeling wired and anxious. That's a simple add-on effect, not a deep chemical one.

There's also a lab-only finding that yerba mate may gently slow the CYP3A4 enzyme, which helps clear the ergotamine part of this medicine. In theory, that could let ergotamine build up a little, and ergotamine can cause nausea, tingling, or cold hands and feet. But this is based only on test-tube research, so I wouldn't lose sleep over it. Mostly, watch the caffeine overlap, and check with your pharmacist if you feel overstimulated.
