Bosentan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Yerba Mate interacts with.

Theoretically, yerba mate might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that yerba mate extract inhibits CYP3A4 enzymes. Theoretically, taking yerba mate may increase levels and adverse effects of CYP3A4 substrates.

Here's the gist with yerba mate and bosentan (Tracleer). Your liver uses an enzyme called CYP3A4 to help clear bosentan from your body. Lab studies suggest yerba mate may gently slow that enzyme down. In theory, that could let a bit more bosentan build up, which might mean more of its usual side effects, like headache, flushing, swelling in the legs or ankles, or feeling unusually tired.

I want to be honest about how thin this evidence is. It comes from test-tube research, not real people, and the interaction is rated as minor. A cup or two of mate is unlikely to cause trouble. Still, since bosentan needs regular liver monitoring anyway, it's smart to mention your mate habit to your pharmacist or doctor, especially if you use concentrated supplement forms.

About Bosentan

What Is Bosentan Used For?

Brand name: Tracleer

Bosentan is used to treat pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs. In adults, it helps improve the ability to exercise and slows how quickly the disease gets worse.

In children aged 3 years and older with idiopathic (no known cause) or congenital (present from birth) PAH, bosentan is used to lower resistance in the lung blood vessels, which is expected to improve exercise ability. Tracleer is one brand approved for both adults and pediatric patients with PAH.
