Guaifenesin, Hydrocodone, Pseudoephedrine falls under STIMULANT DRUGS — the group Yerba Mate interacts with.
Yerba mate contains caffeine. Due to the CNS stimulant effects of the caffeine, concomitant use can increase the risk of adverse effects.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
The piece that matters here is the pseudoephedrine in this combo product. Pseudoephedrine is a decongestant that already revs up your system, and yerba mate brings its own caffeine to the party. Stack the two together and you can pile up the jittery side effects: a racing or pounding heart, feeling wired or anxious, shaky hands, and trouble sleeping. Your blood pressure can also nudge upward.
This is really a matter of two stimulants adding up rather than one changing how the other is broken down, and the warning is based on expert opinion rather than hard studies. An occasional cup of mate is unlikely to cause trouble, but lots of strong mate alongside this medicine could leave you uncomfortably overstimulated. If you notice your heart pounding or feel very jittery, ease back on the mate and check with your pharmacist or doctor.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Guaifenesin, Hydrocodone, Pseudoephedrine also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro research shows that yerba mate extract inhibits CYP3A4 enzymes. Theoretically, taking yerba mate may increase levels and adverse effects of CYP3A4 substrates.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with this combo cough-and-cold product. Of the three ingredients, the hydrocodone is the one that leans on the liver enzyme CYP3A4 to be broken down. Lab-dish studies hint that yerba mate may slow that enzyme a bit, which could in theory let hydrocodone build up slightly. If that happened, you might notice more of its usual effects, like extra drowsiness, dizziness, constipation, or slower breathing.
That said, this is very weak, test-tube-only evidence rated as minor, so a cup or two of mate is unlikely to cause real trouble for most folks. There's also caffeine in mate that can add to the jittery, racing-heart feeling from the pseudoephedrine. If you take this regularly or feel unusually sleepy or wired, check in with your pharmacist or doctor.
