Here's the honest picture: the interaction on file is about caffeine's mild blood-thinning (antiplatelet) effect, and it was matched to the broad group of anticoagulant and antiplatelet drugs. The thing is, selumetinib (Koselugo) isn't really one of those drugs. It's a targeted kinase inhibitor used to shrink nerve tumors in NF1, not a blood thinner. So this particular bleeding concern doesn't fit selumetinib well, and even for true blood thinners the evidence is only theoretical (lab-based, never actually reported in people).

My bigger, practical worry is simpler: yerba mate is a caffeine source, and selumetinib can already cause stomach upset, diarrhea, and tiredness. Stacking on a stimulant can add jitteriness or a racing heart. Since this medicine is for kids, please run any yerba mate use by your child's oncology team or pharmacist first.

In vitro research shows that yerba mate extract inhibits CYP3A4 enzymes. Theoretically, taking yerba mate may increase levels and adverse effects of CYP3A4 substrates.

Here's the gist for your family: selumetinib (Koselugo) is cleared from the body partly by a liver enzyme called CYP3A4. Lab studies suggest yerba mate can slow that enzyme down a little. In theory, that could let selumetinib build up a bit, which might mean more of its usual side effects, things like nausea, diarrhea, mouth sores, skin rash, fatigue, or muscle aches. Selumetinib is not a prodrug, so this would mean stronger effects, not weaker ones.

I want to be honest about the evidence though. This is based only on test-tube work, not real people, and it's rated a minor, possible interaction. An occasional warm mate likely won't cause trouble, but since this drug is used in kids, please run any regular yerba mate use past your child's oncology team or pharmacist first.

What Is Selumetinib Used For?

Brand name: Koselugo

Koselugo (selumetinib) is approved for adults and children 1 year of age and older who have neurofibromatosis type 1 (NF1) — a genetic condition that causes tumors to grow on nerve tissue — when they also have symptomatic, inoperable plexiform neurofibromas (large, deep nerve tumors that cannot be surgically removed and are causing problems).

Both the Koselugo capsule and Koselugo oral granule formulations are approved for this use. The oral granule form is intended for patients who have difficulty swallowing whole capsules.
