Theoretically, chanca piedra might reduce the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that chanca piedra extract increases CYP1A2 activity. Theoretically, chanca piedra might increase metabolism of CYP1A2 substrates and lower serum concentrations. This interaction has not been reported in humans.

Here's the gist for your Triavil (which combines amitriptyline, an antidepressant, with perphenazine, an antipsychotic). Lab studies suggest chanca piedra may rev up an enzyme called CYP1A2, one of the liver pathways that helps break down amitriptyline. If that happened in the body, your system could clear the drug a little faster, which might soften how well it works. You could notice your mood, sleep, or other symptoms slipping back.

That said, this is only seen in test-tube research, not in real people, so it's mostly theoretical. I wouldn't lose sleep over it, but since these are medications you don't want fluctuating, please check with your pharmacist or doctor before adding chanca piedra. Don't change your dose on your own.

Theoretically, use of chanca piedra might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that chanca piedra extract inhibits CYP3A4. Theoretically, chanca piedra might increase the levels of CYP3A4 substrates. This interaction has not been reported in humans.

This combination pill pairs amitriptyline (an older antidepressant) with perphenazine (an antipsychotic). Both rely partly on liver enzymes, including CYP3A4, to break them down. Lab studies hint that chanca piedra can slow that enzyme. If that happens in real life, these medicines could clear a little slower and build up, which might mean more of their usual side effects: drowsiness, dry mouth, constipation, blurry vision, dizziness when you stand, or a faster heartbeat.

Here is the honest part: this is only a test-tube finding, and it has never been reported in actual people, so the real risk is likely small. Still, since these drugs already cause a lot of sedation, it is worth a quick chat with your pharmacist or doctor before adding chanca piedra. Please don't change anything on your own.
