Theoretically, chanca piedra might increase the risk of bleeding when used concomitantly with anticoagulant/antiplatelet drugs.
In vitro research suggests that methyl brevifolincarboxylate, a constituent isolated from chanca piedra, can inhibit platelet aggregation. This effect has not been reported in humans.

Here's the thing to keep an eye on with this combination. Your medication contains aspirin, which already thins the blood a bit by keeping platelets from clumping together. In lab studies, one compound in chanca piedra appeared to slow platelet clumping too. So in theory, stacking the two could add up and raise your chance of bruising, nosebleeds, bleeding gums, or stomach upset and bleeding.

I want to be honest, though: this is only a theoretical concern from test-tube work, and it hasn't actually been seen in people. So please don't panic. It's worth mentioning to your pharmacist or doctor, especially if you notice easy bruising or unusual bleeding, before you start the herb.

Theoretically, chanca piedra might reduce the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that chanca piedra extract increases CYP1A2 activity. Theoretically, chanca piedra might increase metabolism of CYP1A2 substrates and lower serum concentrations. This interaction has not been reported in humans.

Here's how this plays out with your Anacin with Codeine. Of the three ingredients, the caffeine piece is the one handled by the CYP1A2 enzyme. Lab studies (test tube only, not real people) hint that chanca piedra may speed up this enzyme, which could clear caffeine from your body a little faster. In plain terms, that just means the small caffeine boost in this product might wear off sooner. It does not meaningfully change the aspirin (pain and inflammation relief) or the codeine (pain relief and drowsiness) parts.

This is theoretical and has never been reported in actual patients, so I wouldn't lose sleep over it. Still, if you take chanca piedra regularly, mention it to your pharmacist so we can keep an eye on things.

Here's the gist with this combo pill. Lab studies (in test tubes, not people) suggest chanca piedra may slow down a liver enzyme called CYP3A4. That enzyme helps clear the codeine portion of your medicine. If it's slowed, a bit more codeine could hang around, which might mean stronger effects like extra drowsiness, dizziness, constipation, or slowed breathing in larger amounts.

I'll be honest though: this is theoretical and hasn't been seen in actual patients, so I wouldn't lose sleep over it. The aspirin and caffeine parts aren't really driven by this enzyme. Still, since codeine affects breathing, it's worth a quick chat with your pharmacist or doctor before adding chanca piedra, especially if you take this regularly.
