Theoretically, concomitant use of chanca piedra with diuretics might increase diuresis.
Some preliminary clinical research in adults with hypertension shows that chanca piedra has diuretic properties. However, higher quality research in adults with kidney stones shows taking chanca piedra does not increase urine volume when compared with placebo. Until more is known, use cautiously in patients taking diuretic drugs.

Here's the deal with this combo. Your medication already has a built-in 'water pill' (the hydrochlorothiazide part), which makes you pass more urine and helps lower blood pressure. Chanca piedra has a reputation as a mild diuretic too, so in theory stacking the two could add up and push you toward more fluid loss.

If that happened, you might notice things like feeling lightheaded when you stand, increased thirst, more frequent bathroom trips, or muscle cramps from losing salts like potassium and sodium. I want to be straight with you, though: this is a theoretical concern based on how these things work, not strong proof. In fact, better-quality studies didn't show much extra urine output. Still, since you're managing blood pressure, please run it by your pharmacist or doctor before combining them.

Animal research suggests that chanca piedra can decrease blood pressure. However, this effect was not observed in most hypertensive patients treated with chanca piedra for 10 days.

Here's the gist: chanca piedra may have a mild tendency to lower blood pressure on its own. Since your Zestoretic (lisinopril plus hydrochlorothiazide) is already working to bring your numbers down, stacking the two could add up and nudge your blood pressure a little lower than expected.

I want to keep this in perspective, though. This concern comes mostly from animal studies, and in a small human study of people with high blood pressure, chanca piedra didn't actually drop their pressure much over 10 days. So the real-world risk looks small and unlikely. If you do try it, just watch for signs of low blood pressure like dizziness, lightheadedness when you stand, or feeling unusually tired. Check in with your pharmacist or doctor before adding it, and they can help you keep an eye on your readings.

Lisinopril and hydrochlorothiazide tablets — sold under the brand name Zestoretic and as the generic combination — are used to treat high blood pressure (hypertension). Lowering blood pressure reduces the risk of serious cardiovascular events, including strokes and heart attacks.
