Hydrocodone Bitartrate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Chanca Piedra interacts with.

Theoretically, use of chanca piedra might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that chanca piedra extract inhibits CYP3A4. Theoretically, chanca piedra might increase the levels of CYP3A4 substrates. This interaction has not been reported in humans.

Here's the gist for hydrocodone. Your liver uses an enzyme called CYP3A4 to help clear this medicine from your body. In lab studies (not in actual people yet), chanca piedra appears to slow that enzyme down. If that happened in your body, hydrocodone could build up a bit more than usual, which might mean stronger effects like extra drowsiness, dizziness, constipation, or slower breathing.

I want to be honest about how soft this evidence is. It comes from test-tube research only and hasn't been seen in real patients, so this is more of a theoretical caution than a proven problem. Still, because hydrocodone is a strong opioid where even small changes matter, I'd run it by your pharmacist or doctor before adding chanca piedra. Don't change your pain medicine on your own.
