Theoretically, chanca piedra might reduce the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that chanca piedra extract increases CYP1A2 activity. Theoretically, chanca piedra might increase metabolism of CYP1A2 substrates and lower serum concentrations. This interaction has not been reported in humans.

Here's the gist with these two together. Your body uses a liver enzyme called CYP1A2 to help break down imipramine. Lab studies suggest chanca piedra may speed that enzyme up. In theory, that could clear imipramine out a little faster and lower its level in your blood, which might make it work less well for your mood (or for a child's bedwetting).

I want to be honest about the strength of this: it's only test-tube and animal data, and it has never been reported in real people. So this is a theoretical concern, not something I'd panic over. Still, if you notice your depression slipping or symptoms creeping back, loop in your pharmacist or doctor before changing anything.

In vitro research shows that chanca piedra extract inhibits CYP3A4. Theoretically, chanca piedra might increase the levels of CYP3A4 substrates. This interaction has not been reported in humans.

Here's the gist for imipramine (Tofranil). Your body partly uses a liver enzyme called CYP3A4 to clear this antidepressant. In lab dish studies, chanca piedra appears to slow that enzyme down. If the same thing happened in people, imipramine could build up a bit higher than expected, which might mean more of its usual side effects, things like drowsiness, dry mouth, constipation, blurry vision, dizziness when standing, or a faster heartbeat.

Now, the honest part: this is only a theoretical concern from test-tube work, and it hasn't actually been reported in real patients. So please don't panic or change anything on your own. Just give your pharmacist or doctor a quick heads-up before adding chanca piedra so they can keep an eye on how you feel.

About Imipramine

What Is Imipramine Used For?

Brand names: Tofranil, Tofranil PM

Imipramine is used to relieve symptoms of depression. Both Imipramine Hydrochloride tablets and Imipramine Pamoate capsules are approved for this use. Endogenous depression — the kind that seems to arise from within rather than a clear outside cause — tends to respond especially well. It may take one to three weeks before you notice the full benefit.

Imipramine Hydrochloride tablets are also approved as a short-term, add-on treatment to help reduce nighttime bedwetting (nocturnal enuresis) in children aged 6 and older, after a doctor has ruled out physical causes.
