Propranolol Hydrochloride falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Chanca Piedra interacts with.

Theoretically, chanca piedra might reduce the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that chanca piedra extract increases CYP1A2 activity. Theoretically, chanca piedra might increase metabolism of CYP1A2 substrates and lower serum concentrations. This interaction has not been reported in humans.

Here's the gist for propranolol. Your liver uses an enzyme called CYP1A2 to help clear this medicine. Lab and animal studies suggest chanca piedra may speed that enzyme up, which in theory could break propranolol down a little faster and leave less of it in the body. For an infant being treated for a hemangioma, that could mean the medicine works a bit less well.

I want to be straight with you, though: this is only a theoretical concern from test-tube research, and it has never actually been reported in people. So I wouldn't panic. Since propranolol in babies is dosed carefully, please don't add chanca piedra without first checking with your pediatrician or pharmacist.

Theoretically, concomitant use of chanca piedra with antihypertensive drugs might have additive blood pressure lowering effects.
Animal research suggests that chanca piedra can decrease blood pressure. However, this effect was not observed in most hypertensive patients treated with chanca piedra for 10 days.

Here's the gist with chanca piedra and propranolol. This herb may, in theory, nudge blood pressure down a little on its own. Since propranolol is a beta blocker that also lowers blood pressure and slows the heart rate, stacking the two could add up. In a baby being treated for an infantile hemangioma with the oral solution (Hemangeol), the worry would be extra low blood pressure, a slower heart rate, or more sleepiness and sluggish feeding.

That said, the evidence here is thin. This is a theoretical concern from animal studies, and in human research the blood pressure drop mostly didn't show up. I'd still loop in your pediatrician or pharmacist before giving any herb alongside this medication.

What Is Propranolol (Infantile Hemangioma) Used For?

Brand name: Hemangeol

This intravenous (IV) form of propranolol is used in hospital settings to treat serious, life-threatening heart rhythm problems. It works on several types of abnormal rhythms, including those that happen during anesthesia or are caused by drug toxicity.

Specific uses include fast heart rhythms coming from the upper chambers of the heart (supraventricular tachycardia), dangerous rhythms caused by too much digitalis in the body, and certain fast rhythms triggered by excess stress hormones (catecholamines) during surgery.
