Riluzole falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Chanca Piedra interacts with.

Theoretically, chanca piedra might reduce the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that chanca piedra extract increases CYP1A2 activity. Theoretically, chanca piedra might increase metabolism of CYP1A2 substrates and lower serum concentrations. This interaction has not been reported in humans.

Here's the short version for riluzole (Rilutek, Tiglutik, Exservan): your liver clears this drug mainly through an enzyme called CYP1A2. Lab studies suggest chanca piedra may speed that enzyme up. If that holds true in people, your body could break down riluzole a little faster and leave less of it in your blood, which might weaken how well it works for your ALS.

I want to be honest about how soft this evidence is. It comes from test-tube research only and has never been seen in actual patients, so it's more of a theoretical concern than a proven problem. Riluzole isn't a prodrug, so the direction here is reduced effect, not extra side effects. Still, since keeping steady levels matters with this medication, please check with your pharmacist or neurologist before adding chanca piedra.

What Is Riluzole Used For?

Riluzole is approved to treat amyotrophic lateral sclerosis (ALS) — the progressive neurological disease also known as Lou Gehrig's disease. This indication is shared across all available riluzole products, including tablets (Rilutek, riluzole), the dissolving oral film (Exservan), and the oral suspension (Tiglutik).

Tiglutik oral suspension is notable in that it can also be given through a PEG-tube (percutaneous endoscopic gastrostomy tube) for patients who have difficulty swallowing.
