Rivaroxaban falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Chanca Piedra interacts with.

Theoretically, chanca piedra might increase the risk of bleeding when used concomitantly with anticoagulant/antiplatelet drugs.
In vitro research suggests that methyl brevifolincarboxylate, a constituent isolated from chanca piedra, can inhibit platelet aggregation. This effect has not been reported in humans.

Here's the gist: Rivaroxaban (Xarelto) is already a powerful blood thinner that lowers your body's ability to form clots. Chanca piedra has a plant compound that, in lab dish studies, seemed to make platelets (the cells that help your blood stick together) less likely to clump. If that effect carried over into people, pairing it with Xarelto could in theory nudge your bleeding risk a little higher.

I want to be straight with you, though: this is theoretical. It comes from test-tube research and has not been shown in actual patients. Still, since Xarelto can already cause easy bruising, nosebleeds, or bleeding gums, it's worth a quick chat with your pharmacist or doctor before adding chanca piedra, and watch for any unusual bleeding.

Rivaroxaban also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, use of chanca piedra might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that chanca piedra extract inhibits CYP3A4. Theoretically, chanca piedra might increase the levels of CYP3A4 substrates. This interaction has not been reported in humans.

Here's the basic idea: your body clears rivaroxaban (Xarelto) partly through a liver enzyme called CYP3A4. In lab studies (not in actual people), chanca piedra appears to slow that enzyme down. If that happened in your body, rivaroxaban could build up a bit higher than usual. Since this is a blood thinner, the main concern would be more bleeding, things like easy bruising, nosebleeds, bleeding gums, or blood in the urine or stool.

I want to be honest with you, though: this is only a theoretical concern based on test-tube research, and it hasn't been seen in humans. Still, because Xarelto and bleeding are nothing to gamble with, please check with your pharmacist or doctor before pairing them.

About Rivaroxaban

What Is Rivaroxaban Used For?

Brand name: Xarelto

Rivaroxaban tablets are used to reduce the risk of stroke in adults with nonvalvular atrial fibrillation (an irregular heart rhythm), treat deep vein thrombosis (DVT, a blood clot in a vein) and pulmonary embolism (PE, a blood clot in the lungs), and lower the chances of these clots coming back. They also help prevent clots after hip or knee replacement surgery, protect hospitalized patients at risk for clots, and — in combination with aspirin — reduce the risk of major cardiovascular events in adults with coronary artery disease (CAD) or peripheral artery disease (PAD).

Rivaroxaban tablets are also approved for treating VTE (venous thromboembolism, blood clots in veins) and preventing recurrence in pediatric patients from birth to under 18 years, and for preventing clots in children aged 2 and older with congenital heart disease after the Fontan procedure. Rivaroxaban for oral suspension (available as Rivaroxaban Granule and Xarelto oral suspension) is specifically indicated for these two pediatric uses.
