Chanca Piedra and Simvastatin, Sitagliptin

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Chanca Piedra interacts with whole categories of medication. Simvastatin, Sitagliptin falls into 2 of those categories, so it can interact with Chanca Piedra in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

1 of 2

Why this applies to Simvastatin, Sitagliptin

Simvastatin, Sitagliptin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Chanca Piedra interacts with.

Theoretically, use of chanca piedra might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that chanca piedra extract inhibits CYP3A4. Theoretically, chanca piedra might increase the levels of CYP3A4 substrates. This interaction has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Juvisync is a combo pill containing simvastatin (a cholesterol medicine) and sitagliptin (a diabetes medicine). The simvastatin half is the part that matters here. Your body clears simvastatin using a liver enzyme called CYP3A4. In lab dish studies, chanca piedra appears to slow that enzyme down. If that happened in real life, simvastatin could build up in your blood, which raises the chance of statin side effects like muscle aches, weakness, or rarely a more serious muscle problem.

The important caveat: this is only seen in test-tube research and has never been reported in actual people, so it stays theoretical. Still, before adding chanca piedra, check with your pharmacist or doctor so they can watch for any muscle symptoms.

Minor

ANTIDIABETES DRUGS

2 of 2

Simvastatin, Sitagliptin also falls under ANTIDIABETES DRUGS.

Theoretically, concomitant use with antidiabetes drugs might affect glucose control and increase the risk of hypoglycemia.
Animal research suggests that chanca piedra can have hypoglycemic effects. However, a small clinical study in adults with diabetes shows that chanca piedra extract 25 grams orally daily for 1 week does not lower fasting or postprandial blood glucose levels.
Severity Minor Likelihood Unlikely Evidence D

Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Juvisync is a combo pill that pairs sitagliptin (which helps your body lower blood sugar after meals) with simvastatin (a cholesterol-lowering statin). The interaction on file only touches the diabetes side of this medicine. In theory, chanca piedra might nudge your blood sugar a little lower, so stacking it on top of sitagliptin could, on paper, add up to too much of a drop. Signs of low blood sugar include shakiness, sweating, dizziness, confusion, or feeling suddenly weak.

Honestly though, this is a weak, mostly theoretical concern. Animal studies hinted at a sugar-lowering effect, but a small human study found chanca piedra didn't actually change blood sugar. It doesn't affect the simvastatin part. If you'd like to try it, just give your pharmacist or doctor a heads-up first and keep an eye on your readings.

About Chanca Piedra

What is Chanca Piedra?

Chanca Piedra
  • Kidney stones
  • Gallstones
  • Liver support
  • Urinary tract health
  • Digestive complaints

Chanca piedra is a tropical herb traditionally used as a 'stone breaker' for kidney and gallstones, and for liver and urinary health. Human evidence for these uses is limited and mostly small or preliminary, so it should not replace proven treatments. Talk with your pharmacist or doctor before using it, especially if you have a medical condition or take medications.
