Ticagrelor falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Chanca Piedra interacts with.

Theoretically, chanca piedra might increase the risk of bleeding when used concomitantly with anticoagulant/antiplatelet drugs.
In vitro research suggests that methyl brevifolincarboxylate, a constituent isolated from chanca piedra, can inhibit platelet aggregation. This effect has not been reported in humans.

Ticagrelor (Brilinta) is a powerful antiplatelet drug. Its whole job is to keep your platelets from clumping together so clots don't form in your heart or stents. Chanca piedra may have a mild blood-thinning quality of its own. In lab dishes, one of its plant compounds slowed platelet clumping too. So in theory, stacking it on top of ticagrelor could nudge your bleeding risk a little higher.

I want to be honest though: this is only a theoretical concern from test-tube work, and it has not been seen in real people. Still, since ticagrelor already makes you bruise and bleed more easily, watch for things like easy bruising, nosebleeds, bleeding gums, or dark stools. Please check with your pharmacist or doctor before adding it.

Ticagrelor also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, use of chanca piedra might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that chanca piedra extract inhibits CYP3A4. Theoretically, chanca piedra might increase the levels of CYP3A4 substrates. This interaction has not been reported in humans.

Here's the simple version. Your body relies on a liver enzyme called CYP3A4 to clear ticagrelor (Brilinta) out of your system. In lab studies (test tube only, not in people), chanca piedra appears to slow that same enzyme down. If that held true in the body, ticagrelor could build up a bit higher than usual. Since Brilinta is a blood thinner that keeps platelets from clumping, more of it could mean a greater chance of bruising or bleeding, like nosebleeds, bleeding gums, or blood that's slow to stop.

I want to be honest: this is theoretical and hasn't actually been reported in humans, so it's no reason to panic. Still, with a heart medication this important, it's worth a quick chat with your pharmacist or doctor before adding chanca piedra.

About Ticagrelor

What Is Ticagrelor Used For?

Brand name: Brilinta

Ticagrelor tablets are used to lower the risk of cardiovascular death, heart attack (myocardial infarction), and stroke in adults who have had acute coronary syndrome (ACS) — a group of conditions including unstable angina and heart attack — or who have a history of heart attack. It also reduces the risk of stent thrombosis (a blood clot forming in a coronary stent) in people who have been stented for ACS treatment.

Ticagrelor tablets are also used in people with coronary artery disease (CAD) who are at high risk for a first heart attack or stroke, particularly those with type 2 diabetes. Additionally, Brilinta is approved to reduce the risk of stroke in adults with a mild acute ischemic stroke or a high-risk transient ischemic attack (TIA, or "mini-stroke").
