Zileuton falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Chanca Piedra interacts with.
In vitro research shows that chanca piedra extract increases CYP1A2 activity. Theoretically, chanca piedra might increase metabolism of CYP1A2 substrates and lower serum concentrations. This interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Zileuton (Zyflo) and chanca piedra. Your liver uses an enzyme called CYP1A2 to help clear zileuton from your body. In lab studies (not in people), chanca piedra appeared to speed that enzyme up. If that held true in real life, your body might break down zileuton a little faster, leaving less of it around to do its job.
For an asthma controller like this, weaker levels could mean more wheezing, coughing, chest tightness, or shortness of breath over time. I want to stress this is theoretical and hasn't been seen in humans, so don't panic. Just keep an eye on your asthma control and check with your pharmacist or doctor before adding chanca piedra.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

In vitro research shows that chanca piedra extract inhibits CYP3A4. Theoretically, chanca piedra might increase the levels of CYP3A4 substrates. This interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for these two together. Your body relies partly on an enzyme called CYP3A4 to clear zileuton (Zyflo). Lab studies suggest chanca piedra can slow that enzyme down. In theory, that could let zileuton build up a bit more than usual, which might mean stronger effects or a higher chance of side effects like headache, nausea, stomach upset, or muscle aches.
I want to be straight with you, though: this is based only on test-tube research, and it has not been seen in real people. So it's more of a caution than a proven problem. One thing worth noting separately is that zileuton can stress the liver, and chanca piedra is often used for liver and stone support, so it's a good idea to loop in your pharmacist or doctor before combining them.

About Zileuton
What Is Zileuton Used For?
Brand names: Zyflo, Zyflo CR
Zileuton is used for the ongoing prevention and treatment of asthma in adults and adolescents aged 12 and older. It is a maintenance medicine — meaning it is taken regularly to keep asthma under control, not to stop an asthma attack once it has started.
Zyflo (the tablet form) and zileuton extended-release tablets are both approved for this same purpose. Neither form should be used to treat a sudden asthma attack. However, both can be continued during a flare-up of asthma while you also use your rescue inhaler.
