STIMULANT DRUGS
Why this applies to Acetaminophen, Butalbital, Caffeine
Acetaminophen, Butalbital, Caffeine falls under STIMULANT DRUGS — the group Coffee interacts with.
Coffee contains caffeine. Due to the central nervous system (CNS) stimulant effects of caffeine, concomitant use with stimulant drugs can increase the risk of adverse effects.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the simple version: your headache medicine already contains caffeine, right there in the formula along with acetaminophen and butalbital. So when you drink coffee on top of it, you're basically stacking caffeine on caffeine. There's no fancy enzyme trick happening here, just more total stimulant in your system.
What that can feel like is the usual caffeine overload: feeling jittery or shaky, a racing or pounding heartbeat, trouble sleeping, and sometimes anxiety or an upset stomach. This is mostly expert-opinion advice, not a hard danger, but it's a real and predictable effect. If you take this medicine regularly for tension headaches, it's worth chatting with your pharmacist about how much coffee fits comfortably alongside it.

“Alcohol is the interaction I would flag first for anyone taking acetaminophen, butalbital, and caffeine (Esgic, Fiogesic, and others). Drinking raises the risk of liver injury from the acetaminophen component, and 38 of the 252 supplement ingredients on file carry that same liver concern.”

About Coffee
What is Coffee?
- Alertness and reducing fatigue
- Mental focus and concentration
- Athletic performance
- Headache relief
- Weight management
Coffee is a widely consumed beverage made from roasted coffee beans, valued mainly for its caffeine, which boosts alertness and energy. For most healthy adults, moderate coffee intake is generally safe and may have some health benefits, but too much can cause jitteriness, poor sleep, and a faster heartbeat. People who are pregnant, breastfeeding, or sensitive to caffeine should limit their intake and check with a doctor or pharmacist.
About Acetaminophen, Butalbital, and Caffeine
What Is Acetaminophen, Butalbital, and Caffeine Used For?
Brand names: Tecnal, Esgic Plus, Fiogesic, Triad, Repan, Esgic, Fioricet
This three-ingredient combination — butalbital, acetaminophen, and caffeine — is used to relieve the symptoms of tension headache (also called muscle contraction headache). All supported products, including tablets, capsules, and oral solution, share this same approved use.
It is worth knowing that evidence for using this combination to treat repeated, frequent tension headaches over time is limited. Because butalbital can be habit-forming, extended or repeated use is not recommended.
