Ephedrine Sulfate, Hydroxyzine, Theophylline falls under STIMULANT DRUGS — the group Coffee interacts with.
Coffee contains caffeine. Due to the central nervous system (CNS) stimulant effects of caffeine, concomitant use with stimulant drugs can increase the risk of adverse effects.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the simple version: this combination product contains two stimulant-type ingredients. Ephedrine acts like adrenaline, and theophylline is a relative of caffeine itself. When you add the caffeine from your coffee on top, you're basically stacking stimulants. They don't cancel each other out, they add up.
What that can feel like is a racing or pounding heart, jitteriness, trouble sleeping, shakiness, nervousness, or a headache. With theophylline especially, this matters more, because it has a narrow safety window and too much can cause nausea or even an irregular heartbeat. This warning is based on expert opinion rather than hard studies, so a cup or two may be fine for many people, but it's worth running your usual coffee habit by your pharmacist or doctor.

Here's the thing about this combo: theophylline is a stimulant-type medicine for breathing, and it's chemically a close cousin of the caffeine in your coffee. Your body even clears them using some of the same liver pathways. So when you add a lot of coffee on top of theophylline, the caffeine can compete and let theophylline build up a bit higher than expected.
Theophylline already has a narrow safety window, meaning small changes can matter. Too much can bring on a racing or pounding heart, jitteriness, nausea, trouble sleeping, or feeling shaky, and the ephedrine in your product can pile onto that stimulant effect too. A morning cup is usually fine, but heavy coffee intake is where I'd be cautious. Please check with your pharmacist or doctor before changing anything.
