Theoretically, cola nut might reduce the effects of phenobarbital and increase the risk for convulsions.
Cola nut contains caffeine. Animal research suggests that caffeine can decrease the anticonvulsant activity of phenobarbital. However, this effect has not been reported in humans.

Donnatal and similar products combine belladonna alkaloids with phenobarbital, and the phenobarbital part is what we're focused on here. Phenobarbital is used to calm overactive nerve signals, often to help control seizures. Cola nut's main active ingredient is caffeine, which is a mild stimulant that does the opposite, it tends to ramp the nervous system up.

The concern is that caffeine could blunt how well phenobarbital settles things down, possibly making seizures easier to trigger. I want to be honest though: this comes only from animal research and hasn't been seen in people, so it's more theoretical than proven. Since phenobarbital is a medication where steadiness matters, it's worth a quick chat with your pharmacist or doctor before adding a caffeine supplement.
