Ephedrine, Hydroxyzine, Theophylline falls under STIMULANT DRUGS — the group Cola Nut interacts with.
Cola nut contains caffeine. Due to the central nervous system (CNS) stimulant effects of caffeine, concomitant use with stimulant drugs can increase the risk of adverse effects.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
This combination product already packs two stimulants: ephedrine and theophylline. Cola nut adds caffeine on top, and caffeine and theophylline are close chemical cousins. The concern here isn't that cola nut changes how your medicine is broken down. It's simply stacking stimulant on top of stimulant, which can pile on the side effects.
When you double up like this, you may notice a faster or pounding heartbeat, jitteriness, anxiety, trouble sleeping, tremor, or a bump in blood pressure. Theophylline in particular has a narrow safety window, so extra stimulation is worth taking seriously. This caution is based mostly on expert opinion rather than hard studies, but it makes good sense. Please check with your pharmacist or doctor before adding cola nut or other caffeine sources.

Here's the thing to keep in mind with this combo product. It contains theophylline, which your body clears using the same liver pathway that handles caffeine. Since cola nut is basically a caffeine source, taking a lot of it can slow theophylline's breakdown, letting it build up in your blood. Theophylline is a narrow therapeutic index drug, meaning the safe range is small, so even modest changes matter.
If levels climb, you may notice jitteriness, a racing or pounding heart, nausea, trouble sleeping, or feeling shaky. The ephedrine in this product is also a stimulant, so piling caffeine on top can add to that. Before adding a cola nut supplement, please check with your pharmacist or doctor first.

3 of 3Cola nut contains caffeine. There is evidence that using ephedrine with caffeine might increase the risk of serious life-threatening or debilitating adverse effects such as hypertension, myocardial infarction, stroke, seizures, and death.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing about this combination product: it already contains ephedrine (a stimulant) and theophylline (which is closely related to caffeine). Cola nut is basically a natural source of caffeine. So when you add it on top, you're really stacking stimulant on stimulant.
That layering can push your heart rate up, raise your blood pressure, leave you feeling jittery or anxious, and make it hard to sleep. In rare cases, mixing ephedrine with caffeine has been linked to more serious problems like a dangerously high blood pressure, irregular heartbeat, or even seizures. To be fair, this warning leans on anecdotal reports rather than strong studies, but since you're already taking two stimulant-type drugs, it's worth playing it safe.
