Safinamide Mesylate falls under MONOAMINE OXIDASE INHIBITORS (MAOIs) — the group Cola Nut interacts with.

Theoretically, concomitant use might increase the risk of a hypertensive crisis.
Cola nut contains caffeine. Caffeine has been shown to inhibit monoamine oxidase (MAO) A and B in laboratory studies. Concomitant intake of large amounts of caffeine with MAOIs might precipitate a hypertensive crisis. In a case report, a patient that consumed 10-12 cups of caffeinated coffee and took the MAOI tranylcypromine presented with severe hypertension. Hypertension was resolved after the patient switched to drinking decaffeinated coffee.

Here's the gist: Safinamide (Xadago) is an MAO-B inhibitor that helps smooth out your Parkinson's symptoms by raising dopamine. The concern with cola nut is its caffeine. In lab studies caffeine can mildly block MAO enzymes, and there's an old case report where someone drinking 10 to 12 cups of coffee on an MAOI developed a dangerous spike in blood pressure. So in theory, pairing a lot of caffeine from cola nut with safinamide might nudge your blood pressure higher.

I want to keep this in perspective. This is theoretical, based on pharmacology rather than strong human proof, and it really points to large, concentrated amounts. A normal cup of coffee or tea is usually fine. Still, watch for headache, pounding heartbeat, or a flushed feeling, and check with your pharmacist or doctor before using cola nut supplements.
