Phenelzine Sulfate falls under MONOAMINE OXIDASE INHIBITORS (MAOIs) — the group Ephedra interacts with.

Theoretically, concomitant use might increase the risk of serious adverse effects.
Concomitant use of ephedra with MAOIs might increase the risk of hypertension or hypotension, hyperpyrexia, hallucinations, convulsions, rhabdomyolysis, and urinary problems.

Here's the big concern with this pairing. Phenelzine (Nardil) is an MAOI, which means it blocks the enzyme that normally breaks down stimulating brain chemicals like norepinephrine. Ephedra works by flooding your system with those same fight-or-flight signals. Put them together and there's nothing to clear out the buildup, so the stimulant effect can pile up fast.

That can lead to a dangerous spike in blood pressure, pounding heart, headache, sweating, agitation, and in serious cases dangerously high body temperature or seizures. This is a well-recognized, serious combination, not just theoretical. Ephedra is already banned in U.S. supplements, and it should never be mixed with an MAOI. Please talk with your pharmacist or doctor before using any stimulant product while on phenelzine.

What Is Phenelzine Used For?

Phenelzine is used to treat depression — specifically the types described as atypical, nonendogenous, or neurotic depression. This often includes people who have a mix of anxiety and depression, or who have phobic or hypochondriacal features (strong fears about illness or physical symptoms).

It is generally not the first antidepressant a doctor will try. It is best suited for patients who have already tried other antidepressants without enough relief. There is less evidence supporting its use in severe depression with classic biological features (called endogenous depression).
