Why this applies to Esketamine

Esketamine falls under TRICYCLIC ANTIDEPRESSANTS (TCAs) — the group Rauvolfia Vomitoria interacts with.

Theoretically, concomitant use might alter the effects of Rauvolfia vomitoria and increase the risk for adverse effects.
A small clinical study in patients taking TCAs for at least 4 weeks shows that receiving doses of intravenous yohimbine, a constituent of Rauvolfia vomitoria, 2.5-20 mg daily for up to 7 days precipitates severe anxiety, agitation, and tremor. Also, concomitant use of TCAs with Rauvolfia vomitoria may decrease the effects of rauwolfia alkaloids.

Here's the thing to keep in mind if you're considering Rauvolfia vomitoria while using Spravato (esketamine). This plant contains potent compounds, including reserpine and yohimbine-like alkaloids, that stir up your nervous system and affect blood pressure and mood. Esketamine itself can already cause a temporary spike in blood pressure, anxiety, dizziness, and a spaced-out (dissociative) feeling right after your dose. Layering a stimulating, blood-pressure-active herb on top could make those reactions worse, including more agitation, jitteriness, or higher blood pressure readings during your monitoring period.

I'll be honest, this is based mostly on pharmacology and a small study using yohimbine with related antidepressants, not on direct esketamine data. Esketamine is not a prodrug here, so this is about overlapping effects rather than changed metabolism. Please run it past your doctor or pharmacist before combining them.

What Is Esketamine Nasal Spray Used For?

Brand name: Spravato

Spravato (esketamine) nasal spray is approved for two uses in adults. First, it treats treatment-resistant depression (TRD) — depression that has not responded to other antidepressants — either on its own or alongside an oral antidepressant. Second, it treats depressive symptoms in adults with major depressive disorder (MDD) who have acute suicidal ideation or behavior, always used together with an oral antidepressant.

Important limitations: Spravato has not been shown to prevent suicide or reduce suicidal thoughts or behavior directly. Even if you feel better after an early dose, hospitalization may still be needed if your doctor determines it's necessary. Spravato is not approved for use in children or as an anesthetic.
