Mepergan combines two medicines that already slow you down: meperidine, an opioid pain reliever, and promethazine, a sedating antihistamine. Both can make you drowsy, foggy, and slow your breathing on their own. Rauvolfia vomitoria has its own calming, sedative-like effects (mostly seen in animal studies), so stacking it on top could add to that sleepiness.

What this really means: you might feel more groggy, dizzy, or unsteady than usual, and in stronger combinations breathing can become too shallow. To be honest, this is a theoretical concern based on how these things work, not hard human proof, so don't panic. Still, because both pieces of this drug are real sedatives, please check with your doctor or pharmacist before combining them.

Meperidine, Promethazine also falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES.

Theoretically, Rauvolfia vomitoria might increase the levels and clinical effects of CYP2D6 substrates.
Rauvolfia vomitoria contains small amounts of the drug yohimbine. In vitro research shows that yohimbine inhibits CYP2D6 enzyme activity.

This combination product pairs meperidine (a strong opioid pain reliever) with promethazine (an antihistamine that calms nausea and adds sedation). Both are partly cleared by a liver enzyme called CYP2D6. Rauvolfia vomitoria contains a little yohimbine, and lab studies suggest yohimbine can slow that enzyme down. In theory, if the enzyme works slower, these medicines could linger longer and hit harder.

What that might look like in real life: more drowsiness, dizziness, confusion, slowed breathing, or low blood pressure. Keep in mind this is based only on test-tube research, so it's a possible concern, not a proven one. Still, since both drugs are sedating and rauvolfia itself drops blood pressure, please check with your pharmacist or doctor before combining them.
