Clarithromycin falls under CYTOCHROME P450 1A2 (CYP1A2) INHIBITORS — the group Yerba Mate interacts with.
Yerba mate contains caffeine. Caffeine is metabolized by CYP1A2. Theoretically, drugs that inhibit CYP1A2 may decrease the rate of caffeine clearance and increase caffeine levels.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing worth knowing if you're sipping yerba mate while taking clarithromycin. Mate naturally contains caffeine, and your body relies on a liver enzyme called CYP1A2 to clear that caffeine out. Clarithromycin can gently slow that enzyme, so in theory the caffeine may hang around a little longer than usual. That can mean more of the typical caffeine feelings: jitteriness, a racing or pounding heart, trouble sleeping, or feeling wired.
I want to keep this in perspective, though. This is a theoretical interaction based on how these things work on paper, and it's rated as minor. For most people a cup or two won't cause trouble. If you notice you feel extra buzzy, ease back on the mate and check in with your pharmacist or doctor.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Clarithromycin also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
In vitro research shows that yerba mate extract inhibits CYP3A4 enzymes. Theoretically, taking yerba mate may increase levels and adverse effects of CYP3A4 substrates.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the basic idea: your body uses an enzyme called CYP3A4 to help clear clarithromycin (Biaxin). In a lab dish, yerba mate seems to slow that same enzyme down a bit. In theory, that could let clarithromycin linger a little longer, which might nudge up the usual side effects like nausea, stomach upset, a metallic or bitter taste, or diarrhea.
Honestly though, I wouldn't lose sleep over this one. The evidence is only test-tube level, it's rated minor, and a cup or two of mate is unlikely to cause real trouble. Worth knowing that clarithromycin itself is a strong CYP3A4 blocker, so it's usually the bigger player. If you're drinking mate heavily or feeling off, just check with your pharmacist or doctor.

While it is rich in antioxidants and is being studied for weight, blood sugar, and cholesterol, strong human evidence for most health claims is limited. Drinking very hot mate over many years has been linked to higher risk of certain cancers, so enjoy it warm and in moderation.
About Clarithromycin
What Is Clarithromycin Used For?
Brand names: Biaxin, Biaxin XL
Clarithromycin tablets and oral suspension treat a range of mild to moderate bacterial infections in adults and children, including community-acquired pneumonia, acute sinus infections, acute bacterial flare-ups of chronic bronchitis, strep throat and tonsillitis, uncomplicated skin infections, and middle ear infections in children. Tablets are also used in adults to eliminate H. pylori bacteria (in combination with other medicines) to reduce the risk of duodenal ulcer recurrence.
Clarithromycin tablets — but not the extended-release tablet — are also used to treat and prevent disseminated Mycobacterium avium complex (MAC) infections in people with advanced HIV. Clarithromycin extended-release tablets are approved only for acute bacterial exacerbation of chronic bronchitis, acute maxillary sinusitis, and community-acquired pneumonia in adults — not for the other conditions listed above.
