Ivosidenib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Yerba Mate interacts with.

Theoretically, yerba mate might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that yerba mate extract inhibits CYP3A4 enzymes. Theoretically, taking yerba mate may increase levels and adverse effects of CYP3A4 substrates.

Here's the basic idea with ivosidenib (Tibsovo). Your body relies on a liver enzyme called CYP3A4 to break this medication down. Lab studies suggest that yerba mate can mildly slow that same enzyme. In theory, if mate blocks CYP3A4, ivosidenib could clear more slowly and build up a bit, which might nudge up its effects and side effects.

I want to be honest about the strength of this: it comes from test-tube research only, not from people, so it's quite uncertain and rated minor. Still, ivosidenib can affect heart rhythm (a QT-prolongation risk), so I wouldn't shrug it off entirely. Casual mate drinking is likely low concern, but please check with your oncologist or pharmacist before adding concentrated mate supplements.

About Ivosidenib

What Is Ivosidenib Used For?

Brand name: Tibsovo

Tibsovo (ivosidenib) is used to treat adults who have a susceptible IDH1 mutation, confirmed by an FDA-approved test. It treats newly diagnosed acute myeloid leukemia (AML) in adults 75 or older, or those who cannot tolerate intensive chemotherapy — either alone or combined with a medicine called azacitidine. It also treats relapsed or refractory AML (AML that has come back or stopped responding to treatment) and relapsed or refractory myelodysplastic syndromes (MDS).

Tibsovo is also approved for adults with previously treated, locally advanced or metastatic cholangiocarcinoma (bile duct cancer that has spread) who carry an IDH1 mutation.
