Why this applies to Riociguat

Riociguat falls under ANTIHYPERTENSIVE DRUGS — the group Breadfruit interacts with.

Theoretically, concomitant use with antihypertensive drugs might increase the risk of hypotension. Animal research shows that breadfruit leaf extract can decrease heart rate and weaken the force of heart muscle contractions, which may lower blood pressure.
Some antihypertensive drugs include captopril (Capoten), enalapril (Vasotec), losartan (Cozaar), valsartan (Diovan), diltiazem (Cardizem), amlodipine (Norvasc), hydrochlorothiazide (HydroDIURIL), furosemide (Lasix), and many others.

Here's the thing to keep in mind with riociguat (Adempas): its whole job is to relax and widen your blood vessels, which already lowers blood pressure. Concentrated breadfruit leaf products (teas or extracts, not the fruit on your plate) may also nudge blood pressure down a bit. In animal studies they slowed the heart rate and weakened how hard the heart squeezed. Stack that on top of riociguat and you could feel the effects of too-low blood pressure, things like dizziness, lightheadedness, feeling faint when you stand up, or headache.

I'll be honest, this is theoretical and based on lab and animal work, not strong human evidence, so don't panic. Eating breadfruit as food is unlikely to be a problem. But before using any breadfruit leaf supplement, please check with your pharmacist or doctor first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Riociguat also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, breadfruit might inhibit cytochrome P450 3A4 (CYP3A4) and increase levels of drugs metabolized by these enzymes; until more is known, use with caution. In vitro, the breadfruit constituent artocarpin can inhibit CYP3A4 enzymes. Also, animal research shows that breadfruit leaf extract can weakly inhibit CYP3A4 enzymes. However, it is unclear if this inhibitory activity causes a clinically meaningful effect on the metabolism of CYP3A4 substrates.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone), sertraline (Zoloft), and many others.

Here's the situation with breadfruit and riociguat (Adempas). Your liver uses an enzyme called CYP3A4 to help clear riociguat from your body. Lab and animal studies hint that a compound in breadfruit leaves may gently slow that enzyme down. If that happened in people, riociguat could build up a bit, and since it relaxes blood vessels, you might notice more of its blood-pressure-lowering effects like dizziness, lightheadedness, headache, or feeling faint when you stand up.

That said, this is theoretical and only weakly supported, so I don't want to alarm you. Eating breadfruit as a food is very unlikely to matter; concentrated leaf teas or extracts are the bigger question mark. Since riociguat needs careful dosing, please check with your pharmacist or doctor before using any breadfruit supplement.

About Riociguat

What Is Riociguat Used For?

Brand name: Adempas

Riociguat tablets — sold as Adempas and under the generic name Riociguat — are used to treat adults with two types of pulmonary hypertension (high blood pressure in the lungs).

The first is chronic thromboembolic pulmonary hypertension (CTEPH), a condition caused by blood clots in the lung arteries, either after surgery or when surgery is not an option. The second is pulmonary arterial hypertension (PAH), where the arteries carrying blood to the lungs are narrowed or blocked. In both conditions, riociguat helps improve exercise ability and functional status, and in PAH it also helps slow the disease from getting worse.
