Aspirin, Caffeine falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Chanca Piedra interacts with.
In vitro research suggests that methyl brevifolincarboxylate, a constituent isolated from chanca piedra, can inhibit platelet aggregation. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
The aspirin part of this combo (Anacin also has caffeine in it) already thins your blood a bit by making your platelets less sticky. That's how it helps with pain and how it lowers clotting. Chanca piedra may, in theory, do something similar. In lab dish studies, one of its plant compounds slowed platelets from clumping. Stacked on top of aspirin, that could nudge your bleeding risk a little higher.
Here's the honest part: this is only test-tube evidence and hasn't been seen in real people, so don't panic. Still, watch for easy bruising, nosebleeds, bleeding gums, or cuts that are slow to stop. Before adding chanca piedra, run it by your pharmacist or doctor first.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

In vitro research shows that chanca piedra extract increases CYP1A2 activity. Theoretically, chanca piedra might increase metabolism of CYP1A2 substrates and lower serum concentrations. This interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the piece that matters with a product like Anacin: the caffeine part is cleared by a liver enzyme called CYP1A2. In lab studies (not in people), chanca piedra seems to speed that enzyme up. If that held true in the body, your system might break down caffeine a little faster, so you could feel a bit less of its usual lift, like reduced alertness or a weaker boost against that headache.
Honestly, this is very theoretical, based only on test-tube work, and it has never been reported in real patients. The aspirin side isn't really affected by this enzyme. I wouldn't lose sleep over it, but if you take chanca piedra regularly, it's worth a quick chat with your pharmacist or doctor.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

In vitro research shows that chanca piedra extract inhibits CYP3A4. Theoretically, chanca piedra might increase the levels of CYP3A4 substrates. This interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the honest picture with this combo. The lab data suggests chanca piedra can block an enzyme called CYP3A4, which is one pathway your body uses to clear the caffeine portion of Anacin. If that enzyme slows down, caffeine could hang around a bit longer than usual, so in theory you might notice more of its effects: jitteriness, a racing heart, trouble sleeping, or feeling wired.
That said, this is purely a test-tube finding. It has not been seen in real people, and caffeine is actually cleared mostly by a different enzyme (CYP1A2), so the real-world impact is likely small. The aspirin part isn't really affected by this pathway. Neither drug is a prodrug here. If you take Anacin regularly, just mention the chanca piedra to your pharmacist or doctor to be safe.
