Hydrochlorothiazide, Spironolactone falls under DIURETIC DRUGS — the group Chanca Piedra interacts with.

Theoretically, concomitant use of chanca piedra with diuretics might increase diuresis.
Some preliminary clinical research in adults with hypertension shows that chanca piedra has diuretic properties. However, higher quality research in adults with kidney stones shows taking chanca piedra does not increase urine volume when compared with placebo. Until more is known, use cautiously in patients taking diuretic drugs.

Here's the simple version. Your combination pill already does two jobs: hydrochlorothiazide pulls extra fluid off through your kidneys, and spironolactone is a 'water pill' that helps you hold onto potassium. Chanca piedra has been studied as a mild diuretic too, so in theory, stacking it on top could nudge your fluid loss a little further. That might show up as feeling lightheaded, dizzy when you stand up, more frequent urination, or feeling worn out.

I want to be honest about how soft this evidence is. This is a theoretical concern based on how the herb behaves, and better studies in people with kidney stones actually showed no real bump in urine output. So this is more 'keep an eye on it' than a red flag. Before adding chanca piedra, run it by your pharmacist or doctor, especially since spironolactone can also affect your potassium levels.

Animal research suggests that chanca piedra can decrease blood pressure. However, this effect was not observed in most hypertensive patients treated with chanca piedra for 10 days.

Here's the simple version. Your combo pill, hydrochlorothiazide with spironolactone, is a water pill that helps lower blood pressure and remove extra fluid. Chanca piedra has been studied as a mild blood pressure lowerer too, so the theory is that taking both together could nudge your blood pressure down a little more than the medicine alone.

Honestly, this concern is mostly on paper. The blood-pressure effect showed up in animal studies but did not really show up in most people who took chanca piedra for about 10 days. If anything did stack, you might feel lightheaded, dizzy, or tired, especially when standing up fast. Please don't change your dose, just give your pharmacist or doctor a heads-up if you add this herb and watch how you feel.
