Theoretically, cola nut might reduce the effects of phenytoin and increase the risk for convulsions.
Cola nut contains caffeine. Animal research suggests that caffeine can decrease the anticonvulsant activity of phenytoin. The effect does not seem to be related to the seizure threshold-lowering effects of caffeine. However, the exact mechanism of this interaction is unclear.

Here's the gist: mephenytoin (Mesantoin) is an older anti-seizure medicine in the same family as phenytoin. The concern with cola nut is its caffeine. In animal studies, caffeine appeared to weaken the seizure-protecting effect of phenytoin-type drugs. If that carries over to people, it could mean mephenytoin doesn't control seizures as well, which matters since the whole point of the medicine is preventing them.

I want to be honest about the evidence here. This comes from lab and animal research only, and the exact reason it happens isn't clear, so it's more of a theoretical caution than a proven problem. A cup of cola-flavored soda is unlikely to be the issue. Concentrated cola nut supplements or lots of caffeine are where I'd be more careful. Please don't change anything on your own. Just loop in your pharmacist or doctor before adding a cola nut supplement.
