Zileuton falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ephedra interacts with.

Theoretically, ephedra might decrease levels of drugs metabolized by CYP1A2.
Some animal research suggests that ephedra induces CYP1A2 and increases the clearance of CYP1A2 substrates such as caffeine.

Here's the idea behind this pairing. Your body uses an enzyme called CYP1A2 to clear Zileuton out of your system. Animal studies hint that ephedra may speed that enzyme up. If that holds true in people, your body could break down zileuton faster, leaving less of it around to do its job. Since zileuton is a working drug (not a prodrug that needs activating), lower levels could mean weaker asthma control, so you might notice more wheezing, coughing, or chest tightness over time.

I want to be honest that this is theoretical and based only on lab and animal data, so it may not pan out in real life. That said, ephedra itself is banned in U.S. supplements and carries real heart and blood pressure risks. Please don't use it, and check with your pharmacist or doctor about safer options.

Here's the concern with pairing these two. Zileuton is well known for being hard on the liver, which is why your doctor checks your liver enzymes with blood tests, especially during the first few months. Ephedra, the banned stimulant supplement, has also been tied to many reports of liver injury. Put them together and you may be stacking two things that stress the same organ.

I'll be honest with you, this is a theoretical worry based on how each one behaves, not on solid head-to-head studies. Still, watch for warning signs like yellowing skin or eyes, dark urine, belly pain, or unusual tiredness. Honestly, ephedra isn't recommended at all. Please talk with your pharmacist or doctor before combining them.

About Zileuton

What Is Zileuton Used For?

Brand names: Zyflo, Zyflo CR

Zileuton is used for the ongoing prevention and treatment of asthma in adults and adolescents aged 12 and older. It is a maintenance medicine — meaning it is taken regularly to keep asthma under control, not to stop an asthma attack once it has started.

Zyflo (the tablet form) and zileuton extended-release tablets are both approved for this same purpose. Neither form should be used to treat a sudden asthma attack. However, both can be continued during a flare-up of asthma while you also use your rescue inhaler.
