ADENOSINE (Adenocard)

Theoretically, green coffee might decrease the vasodilatory effects of adenosine and interfere with its use prior to stress testing.
Green coffee can contain caffeine. Caffeine is a competitive inhibitor of adenosine at the cellular level. However, caffeine does not seem to affect supplemental adenosine because high interstitial levels of adenosine overcome the antagonistic effects of caffeine. It is recommended that methylxanthines such as caffeine, as well as methylxanthine-containing products, be stopped 24 hours prior to pharmacological stress tests. However, methylxanthines appear more likely to interfere with dipyridamole (Persantine) than adenosine-induced stress testing.

Here's the practical picture. Adenosine is usually given in a hospital or clinic, often during a chemical stress test, where it briefly widens your blood vessels and speeds blood flow to the heart. The caffeine inside green coffee can act like a mild blocker at the same cell receptors adenosine uses, so in theory it could blunt that vasodilating effect and muddy your test results.

In reality, the adenosine dose given during testing is high enough that it usually overrides caffeine, and this concern is mostly theoretical. Still, clinics typically ask you to avoid caffeine and caffeine-containing products (which would include green coffee) for about 24 hours before a stress test. If you have a test coming up, just let your doctor or pharmacist know you take green coffee so they can guide the timing.
