Palbociclib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP3A4; however, a small clinical study found no effect.
In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.

Here's the gist for Ibrance (palbociclib): your body clears it using a liver enzyme called CYP3A4. In a test tube, red clover looked like it might slow that enzyme down, which in theory could let palbociclib build up a bit and bump up its usual side effects (think low white blood cell counts, fatigue, nausea, or mouth sores). But here's the reassuring part: when researchers actually gave red clover to people and checked another CYP3A4 drug, they saw no real change.

So this is a mostly theoretical, low-likelihood concern. Still, palbociclib is serious cancer therapy, so I wouldn't add red clover on your own. Loop in your oncologist or pharmacist first, especially since red clover acts like a weak estrogen and your breast cancer is hormone-driven.

About Palbociclib

What Is Palbociclib Used For?

Brand name: Ibrance

Palbociclib (brand name Ibrance) is used to treat adults with hormone receptor (HR)-positive, HER2-negative advanced or metastatic breast cancer. It is always given in combination with other medicines — either an aromatase inhibitor as a first hormonal treatment, or with fulvestrant after the cancer has progressed on prior hormonal therapy. It is also used with inavolisib and fulvestrant for a specific subtype (PIK3CA-mutated, endocrine-resistant) of this cancer confirmed by an FDA-authorized test.

Ibrance is also approved in combination with trastuzumab (with or without pertuzumab) and endocrine therapy for maintenance treatment of adults with HR-positive, HER2-positive locally advanced or metastatic breast cancer, following initial induction treatment.
