Theanine and Levomethadyl

Here is the interaction we found between these two, with the evidence behind it.

Minor

CNS DEPRESSANTS

Why this applies to Levomethadyl

Levomethadyl falls under CNS DEPRESSANTS — the group Theanine interacts with.

Theoretically, theanine might have additive sedative effects when used in conjunction with CNS depressants. However, it is unclear if this concern is clinically relevant.
Theoretically, theanine may compete with glutamate and/or increase plasma gamma-aminobutyric acid (GABA) levels, which could cause CNS depression. In one clinical study, some subjects taking oral theanine reported drowsiness.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Levomethadyl (Orlaam) is a long-acting opioid that was used to treat opioid dependence. Like other opioids, it slows down your central nervous system, which can leave you sleepy, mentally foggy, and in higher doses can slow your breathing. Theanine, the calming amino acid found in tea, may gently nudge things like GABA in the brain in a relaxing direction.

On paper, stacking these two could add up to a little extra drowsiness. But honestly, this is a theoretical concern only, based on how each works, not on real cases of harm. The far bigger sedation risk here comes from the opioid itself, not the theanine. Still, since this is a serious medication, it's worth a quick word with your pharmacist or doctor before adding any supplement.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Levomethadyl page

“Alcohol is the single clearest hazard here. Beer and wine carry major ratings backed by human studies, because levomethadyl (Orlaam), a long-acting opioid used to treat opioid dependence, becomes far more sedating alongside anything else that slows the brain.”

About Theanine

What is Theanine?

Theanine
  • Anxiety and stress
  • Relaxation and calm
  • Sleep support
  • Focus and cognitive function

Theanine (usually L-theanine) is an amino acid found naturally in tea leaves that many people take to feel calmer and less stressed without strong drowsiness. Early research suggests it may help with relaxation and stress, but evidence for most uses is still limited. It appears generally well tolerated for short-term use, but check with your pharmacist or doctor before starting it.
