Tree Turmeric and Methadone

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Tree Turmeric interacts with whole categories of medication. Methadone falls into 3 of those categories, so it can interact with Tree Turmeric in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

CNS DEPRESSANTS

1 of 3

Why this applies to Methadone

Methadone falls under CNS DEPRESSANTS — the group Tree Turmeric interacts with.

Theoretically, use of tree turmeric along with CNS depressants might increase the risk of additive therapeutic and adverse effects.
Animal research suggests that berberine, a constituent of tree turmeric, can have sedative effects.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: methadone is already a strong medicine that can slow your breathing and make you very sleepy. Tree turmeric (which contains berberine) has shown some calming, sedative effects in animal studies. The worry is that putting the two together could stack those effects, so you might feel groggier, more dizzy, or more sedated than you expect.

I want to be honest about the evidence here. This is mostly theoretical and comes from lab and animal work, not solid human studies, so it may not amount to much in real life. Still, methadone is nothing to gamble with, so don't add a berberine or tree turmeric supplement without first checking with your pharmacist or doctor, especially if you notice extra drowsiness.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

2 of 3

Methadone also falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES.

Theoretically, tree turmeric might increase the levels and clinical effects of drugs metabolized by CYP2D6.
In vitro research and preliminary clinical evidence suggest that berberine, a constituent of tree turmeric, can inhibit CYP2D6.
Severity Moderate Likelihood Possible Evidence B

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: the berberine in tree turmeric may gently block an enzyme called CYP2D6, which is one of the pathways your body uses to break down methadone. If that pathway slows down, more methadone can linger in your system than usual. Methadone is an active drug (not a prodrug that needs switching on), so blocking its breakdown tends to boost its effects rather than weaken them.

In real terms, that could mean more drowsiness, slowed breathing, constipation, or feeling lightheaded. Methadone also has a narrow safety margin and can affect heart rhythm, so even modest shifts deserve respect. Honestly, this concern is mostly theoretical and based on limited evidence, but please don't add tree turmeric without first checking with your pharmacist or doctor.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

3 of 3

Methadone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, tree turmeric might increase the levels and clinical effects of drugs that are substrates of CYP3A4.
In vitro research and preliminary clinical evidence suggest that berberine, a constituent of tree turmeric, moderately inhibits CYP3A4.
Severity Moderate Likelihood Probable Evidence A

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: your body clears methadone partly through a liver enzyme called CYP3A4. The berberine in tree turmeric can slow that enzyme down. When methadone breaks down more slowly, more of it can build up in your system, which means stronger effects and a higher chance of side effects like deep drowsiness, confusion, very slow or shallow breathing, and constipation.

Methadone is a drug where small changes really matter, since too much can dangerously slow your breathing, and it can also affect your heart rhythm. The data here is mostly lab and early human work, so this isn't a certainty, but I'd treat it seriously. Please don't start or stop anything on your own. Talk with your pharmacist or your methadone clinic prescriber before adding tree turmeric.

  • Skin and eye conditions
  • Antimicrobial uses
  • Liver support

Tree turmeric (Berberis aristata) is a shrub used in traditional Indian (Ayurvedic) medicine, valued mainly for its berberine content. About Methadone

What Is Methadone Used For?

Brand name: Dolophine

Methadone treats severe and persistent pain that requires daily, around-the-clock, long-term opioid treatment and cannot be managed adequately with other options. It is available in several forms for this use, including oral solution, oral tablets, oral concentrate, and injectable forms (intravenous, intramuscular, and subcutaneous). Injectable methadone is not approved for outpatient pain treatment — it is reserved for hospitalized patients unable to take oral medication.

Methadone is also used for the detoxification and maintenance treatment of opioid addiction (dependence on heroin or other morphine-like drugs). Oral forms — including tablets, dispersible tablets (such as Methadose Dispersible and DISKETS), and oral concentrates (such as Methadone Hydrochloride Intensol™, Methadose, and others) — are approved for this use. By law, methadone for opioid addiction treatment must be dispensed only by certified opioid treatment programs. Injectable methadone is not approved for outpatient addiction treatment.
