Artemisinin is generally well tolerated when taken by mouth. The most common side effects are nausea and vomiting.

Rare but serious side effects include liver damage (hepatotoxicity) — one documented case showed hepatitis with elevated liver enzymes after someone took 600 mg daily for 10 days, though the link wasn't certain. A very rare allergic reaction (rash and cough) has been reported; in people allergic to Sweet Annie, taking artemisinin under the tongue may cause numbness or throat itching.

The product is typically used short-term, and quality and dose vary widely across brands, so it should not replace proven treatments. Do not use artemisinin during pregnancy because of possible harm to the developing baby.

Cytochrome P450 2B6 (Cyp2B6) Substrates

Sweet Annie may alter plasma levels and clinical effects of drugs metabolized by CYP2B6.
In vitro research shows that the Sweet Annie constituent artemisinin induces CYP2B6, possibly increasing CYP2B6 activity by 1.6-fold. However, Sweet Annie extract seems to inhibit the activity of CYP2B6 in vitro, suggesting that other constituents of Sweet Annie play a role in its effects on the overall activity of this enzyme. More information is needed to determine whether taking Sweet Annie extract affects the metabolism of CYP2B6 substrates.
