Chlorthalidone falls under DIURETIC DRUGS — the group Chanca Piedra interacts with.

Theoretically, concomitant use of chanca piedra with diuretics might increase diuresis.
Some preliminary clinical research in adults with hypertension shows that chanca piedra has diuretic properties. However, higher quality research in adults with kidney stones shows taking chanca piedra does not increase urine volume when compared with placebo. Until more is known, use cautiously in patients taking diuretic drugs.

Here's the gist: chanca piedra has been called a 'stone breaker,' and a little early research hints it may have its own mild water-pill effect. Since chlorthalidone is already a diuretic that makes you pee out extra salt and water, the worry is that the two together could nudge you toward losing a bit more fluid than intended.

In real life, that might show up as feeling lightheaded when you stand, more thirst, muscle cramps, or low potassium. That said, this is a theoretical concern based on how the herb might behave, and one better-quality study in people with kidney stones found it did not actually boost urine output. So I wouldn't panic, just loop in your pharmacist or doctor before combining them, especially if you already run low on fluids or potassium.

Theoretically, concomitant use of chanca piedra with antihypertensive drugs might have additive blood pressure lowering effects.
Animal research suggests that chanca piedra can decrease blood pressure. However, this effect was not observed in most hypertensive patients treated with chanca piedra for 10 days.

Here's the simple version: chlorthalidone already works to lower your blood pressure by helping your body shed extra salt and water. Some animal studies hint that chanca piedra may nudge blood pressure down a little too. If both lean the same direction, you could in theory see a slightly bigger drop than expected.

The good news is this is mostly a theoretical concern. When researchers gave chanca piedra to people with high blood pressure for ten days, they did not really see that extra lowering. So a meaningful interaction is unlikely. Still, if you start it, keep an eye out for dizziness, lightheadedness when standing, or feeling unusually tired, and check in with your pharmacist or doctor before adding it.

What Is Chlorthalidone Used For?

Brand names: Hygroton, Thalitone

Chlorthalidone is used to treat high blood pressure (hypertension) in adults. Lowering blood pressure reduces the risk of serious cardiovascular events, especially strokes and heart attacks. It can be used alone or combined with other blood pressure medicines.

Chlorthalidone is also used as an add-on treatment for edema (fluid buildup) caused by heart failure, liver cirrhosis, and kidney conditions including nephrotic syndrome, acute glomerulonephritis, and chronic renal failure. Thalitone is additionally indicated for edema associated with heart failure, liver cirrhosis, and renal disease including nephrotic syndrome.
