Here's the concern with combining ephedra and either phenobarbital or phenytoin (sometimes sold together as Dilantin with PB). Both of these seizure medicines can put some stress on the liver, and ephedra has been tied to a number of reported cases of liver injury. The worry is that using them together could add up and make liver problems more likely. Warning signs of liver trouble include yellowing of the skin or eyes, dark urine, belly pain, nausea, or unusual tiredness.

I'll be honest, this particular interaction is theoretical, based on how each one behaves rather than proven cases of the combination. That said, ephedra is banned in U.S. supplements for good reason, and I wouldn't recommend it at all. Please talk with your pharmacist or doctor about safer choices.

Here's the basic idea with this combination. Phenobarbital and phenytoin (Dilantin with PB) work to calm overactive nerve signals and prevent seizures. Ephedra is a strong stimulant that revs up the nervous system, and it has actually been linked to seizures in some people. So in theory, ephedra could work against your seizure medicine and make it harder to keep seizures controlled.

I'll be honest: this is based on how these substances act in the body, not on solid clinical studies, so the real-world risk isn't well proven. But ephedra is banned in U.S. supplements for good reason, and it's a poor mix for anyone managing a seizure disorder. Please don't change your medication, and talk with your pharmacist or doctor before using any stimulant product.
