Why this applies to Perphenazine
Perphenazine falls under PHENOTHIAZINES — the group Green Coffee interacts with.
Green coffee can contain caffeine. Phenothiazines can reduce the metabolism of caffeine by inhibiting cytochrome P450 1A2 (CYP1A2).
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with green coffee and perphenazine (Trilafon). The concern isn't really about the perphenazine building up. Instead, perphenazine can mildly slow down a liver enzyme called CYP1A2, which is the same one your body uses to clear caffeine. Green coffee naturally contains caffeine, so in theory the caffeine could hang around longer and feel stronger than usual.
In plain terms, you might notice more jitteriness, a racing heart, trouble sleeping, or feeling on edge. The good news is this is rated minor and is based only on lab-type evidence, so for most people it's not a big deal. A small everyday cup is unlikely to cause trouble, but concentrated green coffee supplements pack more caffeine. If you feel wired, check in with your pharmacist or doctor.

Perphenazine has not been shown effective for managing behavioral problems in patients with intellectual disabilities, and it is not approved for dementia-related psychosis in elderly patients.
