Osimertinib Mesylate falls under CYTOCHROME P450 1A2 (CYP1A2) INHIBITORS — the group Guarana interacts with.

Theoretically, concomitant use might increase the levels and adverse effects of caffeine.
Guarana contains caffeine. Caffeine is metabolized by the CYP1A2 enzyme. Theoretically, drugs that inhibit CYP1A2 may decrease the rate of caffeine clearance and increase caffeine levels.

Here's the key thing to understand: this interaction is really about the caffeine in guarana, not about your cancer medicine itself. Osimertinib (Tagrisso) can mildly block CYP1A2, the liver enzyme that clears caffeine out of your system. When that enzyme is slowed down, caffeine can hang around longer than usual, so a guarana supplement might hit you harder than you'd expect.

In plain terms, you could feel more jittery, anxious, restless, or have a racing heart and trouble sleeping. This is only a theoretical concern based on lab studies, and it's rated minor, so it's not dangerous for most people. Tagrisso isn't a prodrug, so its cancer-fighting power isn't changed here. Still, since heart effects matter on this medicine, check with your pharmacist or oncologist before adding guarana.

What Is Osimertinib Used For?

Brand name: Tagrisso

Tagrisso is used to treat adults with non-small cell lung cancer (NSCLC) whose tumors have specific EGFR mutations — either an exon 19 deletion or an exon 21 L858R mutation — confirmed by an FDA-approved test. It is used in several settings: after surgery to reduce the risk of cancer returning (adjuvant therapy), for locally advanced stage III NSCLC that has not progressed after chemoradiation, and as a first-line treatment for locally advanced or metastatic NSCLC (including in combination with pemetrexed and platinum-based chemotherapy).

Tagrisso is also approved for adults with metastatic NSCLC that carries a specific resistance mutation called T790M, when the cancer has stopped responding to another EGFR-targeted therapy.
