Here's the simple version: Actifed already contains pseudoephedrine, a decongestant that acts like a mild stimulant on its own. It can bump up your heart rate, raise blood pressure a little, and leave some people feeling jittery or wired. Guarana is basically a concentrated source of caffeine, which is another stimulant. Stacking the two means you're piling stimulant on top of stimulant.

When you combine them, you may notice more of the same effects: a racing or pounding heart, restlessness, shakiness, trouble sleeping, or feeling anxious. This is mostly a theoretical concern based on how both work, not strong study data, so don't panic if you've taken them together once. Still, if you have heart issues, high blood pressure, or anxiety, talk with your pharmacist or doctor before pairing them.
