Treprostinil falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Guarana interacts with.

Theoretically, guarana may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
In vitro and animal research suggests that guarana extract can inhibit platelet aggregation. This effect may be due to the caffeine in guarana, which is also reported to have antiplatelet activity. This interaction has not been reported in humans.

Here's the thing to know about pairing guarana with treprostinil (Orenitram). Treprostinil itself works to relax and widen blood vessels in your lungs, and as part of how it works it can also make your platelets a little less sticky, which slightly nudges up bleeding risk on its own. Guarana is basically a concentrated source of caffeine, and lab and animal studies hint that caffeine may also make platelets clump together less easily.

So in theory, stacking the two could add up and raise your chance of bruising or bleeding more easily. I want to be honest though: this has only been seen in test tubes and animals, never confirmed in people, so it's a soft, theoretical concern. A coffee-sized amount of caffeine is unlikely to matter much; high-dose guarana energy products are where I'd be more cautious. Guarana's caffeine can also bump up your heart rate, which isn't ideal alongside a lung-pressure medicine. Please run it by your pharmacist or doctor before combining them.

About Treprostinil

What Is Treprostinil Used For?

Treprostinil treats pulmonary arterial hypertension (PAH; WHO Group 1) — high blood pressure in the arteries of the lungs. Depending on the product and route, it is used to improve exercise ability, delay disease progression, or reduce symptoms tied to physical activity. Remodulin and treprostinil injection products are also indicated for patients who need to transition away from another PAH medicine called epoprostenol.

Tyvaso, Tyvaso DPI, and Yutrepia (inhaled forms) are additionally approved to treat pulmonary hypertension associated with interstitial lung disease (PH-ILD; WHO Group 3) — lung-pressure problems caused by scarring or inflammation in the lung tissue itself. Orenitram (extended-release oral tablet) is specifically indicated to delay disease progression and improve exercise capacity in PAH.
