Why this applies to Alpelisib
Alpelisib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Hemp interacts with.
In vitro research shows that hemp induces CYP3A4 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for Piqray (alpelisib): your body partly clears it using a liver enzyme called CYP3A4. Lab studies suggest that hemp might speed that enzyme up a bit. If that happened in real life, your body could break alpelisib down faster, leaving lower levels in your blood and potentially making it work less well against your cancer. Alpelisib is not a prodrug, so faster clearing means less benefit, not more.
The good news is this is only a theoretical concern based on test-tube research, not human studies, so I wouldn't panic over hemp seeds or oil in food. Still, since alpelisib is a cancer drug where every bit of effect counts, please run any concentrated hemp supplement past your oncologist or pharmacist first.

About Alpelisib
What Is Alpelisib Used For?
Brand name: Piqray
Alpelisib is approved for two distinct uses, each tied to a specific brand. Piqray is used in combination with another drug called fulvestrant to treat adults with hormone receptor (HR)-positive, HER2-negative, advanced or metastatic breast cancer whose tumors carry a PIK3CA mutation — and only after the cancer has progressed on or after a hormone-based treatment. A companion diagnostic test is required before starting Piqray.
Vijoice is used to treat adults and children 2 years of age and older who have severe manifestations of PIK3CA-Related Overgrowth Spectrum (PROS) — a rare condition where PIK3CA gene mutations cause abnormal tissue, vascular, and bone overgrowth — and who need systemic (whole-body) therapy. Vijoice's approval for PROS was granted under the FDA's accelerated approval pathway based on response data, and continued approval may depend on confirmatory trials.
