Cilostazol falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Hemp interacts with.

Theoretically, hemp seed might increase the risk of bleeding when used concomitantly with anticoagulant/antiplatelet drugs.
In animal research, hemp seed at 5% of the diet inhibits platelet aggregation in vitro. However, in human research, taking hemp seed oil 2 grams daily for 12 weeks does not inhibit the aggregation of platelets in vitro.

Here's the simple version: cilostazol (Pletal) is already an antiplatelet medicine, meaning it makes your platelets less likely to clump and form clots so blood flows more freely to your legs. The concern with hemp is that, in theory, it might also nudge platelets in that same direction. Stack the two together and you could, in principle, raise your chances of bruising or bleeding more easily.

Now, the honest part: this worry comes from lab and animal studies only. In actual human research, hemp seed oil at 2 grams a day for 12 weeks did not meaningfully affect platelets. So everyday hemp seeds or hemp foods are very unlikely to cause trouble. Still, if you plan to use concentrated hemp supplements, give your pharmacist or doctor a quick heads-up first.

Theoretically, hemp might decrease the levels and clinical effects of CYP3A4 substrates.

Here's the gist with hemp and cilostazol (Pletal). Your body relies on a liver enzyme called CYP3A4 to break down cilostazol. Early lab research hints that hemp might rev up that enzyme a bit, which could clear cilostazol out of your system a little faster. In theory, that might make it work less well, so you could notice less improvement in your leg cramping and walking distance from your intermittent claudication.

I want to be honest, though: this is rated minor and comes only from test-tube studies, not real people. Everyday hemp seeds or hemp oil in food are very unlikely to cause a problem. If you're considering concentrated hemp supplements, just run it by your pharmacist or doctor first.
