Hemp and Indinavir

Here is the interaction we found between these two, with the evidence behind it.

Minor

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Indinavir

Indinavir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Hemp interacts with.

Theoretically, hemp might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that hemp induces CYP3A4 enzymes.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for indinavir (Crixivan). Your body clears this HIV medicine using a liver enzyme called CYP3A4. Some early lab research hints that hemp may speed up that enzyme, which in theory could push indinavir out of your system a little faster and lower its levels in your blood.

Why does that matter? With HIV protease inhibitors, you need steady drug levels to keep the virus suppressed. If levels dip too low, the medicine may not work as well over time. That said, this concern comes only from test-tube studies, not from people, and it's rated a minor, possible interaction. Eating hemp seeds in food is unlikely to be a problem. Before using concentrated hemp supplements, just check with your pharmacist or HIV provider.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

This page covers Hemp only. If you take other supplements — a multivitamin, a sleep aid, something for joints — the Indinavir page lists 150 supplements it has a documented interaction with, worst first, each one explained.

The most serious there right now: Activated Charcoal (major), Grapefruit (major), St. John's Wort (major), Acacia Rigidula (moderate) and Adrafinil (moderate).

By severity: 3 major · 109 moderate · 38 minor.

From our pharmacist’s overview on the Indinavir page

“St. John's wort can drop indinavir (Crixivan) levels far enough to cause HIV treatment failure and viral resistance, which is why this list calls for real care before adding any supplement.”

About Hemp

What is Hemp?

Hemp
  • Plant-based protein source
  • Heart health support
  • Skin and hair health
  • Reducing inflammation
  • General nutrition (omega fatty acids)

Hemp seeds and hemp seed oil are nutritious foods rich in protein, fiber, and healthy omega-3 and omega-6 fatty acids, and they are generally safe for most people as part of the diet. While the nutrition is real, strong human evidence that hemp supplements treat specific medical conditions is limited. Talk with your pharmacist or doctor before using hemp products for a health concern, especially if you are pregnant or breastfeeding.

About Indinavir

What Is Indinavir Used For?

Brand name: Crixivan

  • Indinavir is used along with other medications to treat human immunodeficiency virus (HIV) infection. Indinavir is in a class of medications called protease inhibitors. It works by decreasing the amount of HIV in the blood. Although indinavir does not cure HIV, it may decrease your chance of developing acquired immunodeficiency syndrome (AIDS) and HIV-related illnesses such as serious infections or cancer. Taking these medications along with practicing safer sex and making other lifestyle changes may decrease the risk of transmitting the HIV virus to other people.
