ANTIHYPERTENSIVE DRUGS

Macitentan, Tadalafil falls under ANTIHYPERTENSIVE DRUGS — the group Hemp interacts with.
In a hypertensive animal model, hemp seed protein hydrolysate reduced systolic blood pressure by a mechanism possibly involving the inhibition of renin and angiotensin converting enzyme (ACE) activities. However, there was no effect of hemp seed protein on blood pressure in normotensive animals. Furthermore, hempseed oil consumption does not seem to reduce blood pressure in humans.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Opsynvi combines two medicines that both work to lower pressure in the lungs and open blood vessels, so they can nudge your overall blood pressure down. The concern here is simple addition: hemp seed protein might gently lower blood pressure too, so in theory the two together could drop your pressure a bit more than expected. That could mean feeling dizzy, lightheaded, or faint, especially when standing up quickly.
Honestly though, this is very weak evidence. The blood-pressure effect showed up only in animal studies using concentrated hemp seed protein, and hemp oil didn't lower pressure in people. Eating hemp seeds as food is very unlikely to cause a problem. If you use strong hemp supplements, just mention it to your pharmacist or doctor.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

In vitro research shows that hemp induces CYP3A4 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Opsynvi combines two medicines, macitentan and tadalafil, and both are broken down by a liver enzyme called CYP3A4. Some lab studies hint that hemp might speed that enzyme up a little. If that happened in the body, it could nudge the levels of these two drugs down, which in theory might make them work slightly less well for your pulmonary hypertension.
Here's my honest take: this is a very weak, theoretical concern. It comes from test-tube research only, not from people, and it's rated as a minor interaction. Everyday hemp seeds or hemp oil in food are unlikely to cause any trouble. If you're using concentrated hemp supplements, just mention it to your pharmacist or doctor so they can keep an eye on how well your Opsynvi is controlling your symptoms.
