Hemp and Pomalidomide

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Hemp interacts with whole categories of medication. Pomalidomide falls into 2 of those categories, so it can interact with Hemp in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Minor).

Minor

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

1 of 2

Why this applies to Pomalidomide

Pomalidomide falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Hemp interacts with.

Theoretically, hemp might decrease the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that hemp induces CYP1A2 enzymes.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the gist for Pomalidomide (Pomalyst). Your body partly relies on an enzyme called CYP1A2 to break this medicine down. Lab studies (in test tubes, not people) hint that hemp may speed up that enzyme. If that happened in real life, your body could clear pomalidomide a little faster, which in theory might make it slightly less effective against your myeloma or Kaposi's sarcoma. Pomalidomide is the active drug, not a prodrug, so faster breakdown means less of it sticking around, not more.

Honestly, this is theoretical and weak evidence, and food amounts of hemp seeds are unlikely to matter. Still, because this is a serious cancer drug, please run any hemp supplement past your oncology team or pharmacist first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Pomalidomide also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, hemp might decrease the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that hemp induces CYP3A4 enzymes.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the basic idea with hemp and pomalidomide (Pomalyst). Your liver uses an enzyme called CYP3A4 to help break down pomalidomide, and lab studies hint that hemp might rev that enzyme up a bit. If that happened in the body, your system could clear the drug a little faster, which in theory might make it work less well against your multiple myeloma or Kaposi's sarcoma.

I want to be honest about how thin this evidence is. It comes only from test-tube research, not from people, so it's labeled minor and theoretical. Everyday hemp foods like hemp seeds are very unlikely to matter. Still, because pomalidomide treats serious cancer, please run any concentrated hemp supplement past your oncology team or pharmacist first.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

About Hemp

What is Hemp?

Hemp
  • Plant-based protein source
  • Heart health support
  • Skin and hair health
  • Reducing inflammation
  • General nutrition (omega fatty acids)

Hemp seeds and hemp seed oil are nutritious foods rich in protein, fiber, and healthy omega-3 and omega-6 fatty acids, and they are generally safe for most people as part of the diet. While the nutrition is real, strong human evidence that hemp supplements treat specific medical conditions is limited. Talk with your pharmacist or doctor before using hemp products for a health concern, especially if you are pregnant or breastfeeding.

About Pomalidomide

What Is Pomalidomide Used For?

Brand name: Pomalyst

Pomalidomide capsules are used in adults with multiple myeloma (a blood cancer affecting plasma cells) who have already tried at least two prior treatments — including lenalidomide and a proteasome inhibitor — and whose disease has continued to progress. It is given in combination with dexamethasone for this use.

Pomalidomide capsules are also used to treat Kaposi sarcoma (KS) — a cancer that can cause lesions on the skin and in internal organs — both in adults with AIDS-related KS after antiretroviral therapy has stopped working, and in adults with KS who are HIV-negative. The KS indication was approved based on response rate; ongoing studies are confirming long-term benefit.
