Here's the simple version: Ziac already lowers your blood pressure two ways. The bisoprolol is a beta blocker that slows your heart and eases its workload, and the hydrochlorothiazide is a water pill that helps your body shed extra fluid and salt. Rauvolfia vomitoria contains a small amount of reserpine, which also brings blood pressure down. Stack them together and the lowering effects can add up.

If that happens, you might feel dizzy, lightheaded, tired, or faint, especially when you stand up quickly. This concern is theoretical, based on how these compounds work rather than firm studies, so don't panic. Still, since this herb is potent, please check with your pharmacist or doctor before combining them.

Theoretically, Rauvolfia vomitoria might increase the levels and clinical effects of CYP2D6 substrates.
Rauvolfia vomitoria contains small amounts of the drug yohimbine. In vitro research shows that yohimbine inhibits CYP2D6 enzyme activity.

Here's the basic idea: Ziac combines bisoprolol (a beta-blocker) with hydrochlorothiazide (a water pill). The bisoprolol part is partly cleared by a liver enzyme called CYP2D6. Rauvolfia vomitoria contains a little yohimbine, and lab studies suggest yohimbine can slow that enzyme down. In theory, that could let bisoprolol build up a bit, leading to stronger effects like a slower heart rate, more tiredness, dizziness, or lightheadedness when standing.

I want to keep this in perspective. This concern comes only from test-tube research, not from real patients, so it's theoretical. That said, rauvolfia also has its own strong effects on blood pressure and mood, so combining the two isn't something to do casually. Please check with your doctor or pharmacist before mixing them.
