Dorzolamide, Timolol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Rauvolfia Vomitoria interacts with.

Theoretically, Rauvolfia vomitoria might increase the levels and clinical effects of CYP2D6 substrates.
Rauvolfia vomitoria contains small amounts of the drug yohimbine. In vitro research shows that yohimbine inhibits CYP2D6 enzyme activity.

Your Cosopt eye drops combine dorzolamide with timolol, and timolol is the piece that matters here. Timolol is a beta blocker that your liver clears using an enzyme called CYP2D6. Rauvolfia vomitoria contains a small amount of yohimbine, which lab studies suggest can slow that same enzyme down. In theory, that could let more timolol build up in your body, nudging up its usual effects like a slower heart rate, lower blood pressure, tiredness, or dizziness.

Honestly though, this is a theoretical concern based only on test-tube research, and eye drops deliver a small dose. It's likely minor for most people, but Rauvolfia itself strongly lowers blood pressure, so the bigger worry is the combined drop in pressure and pulse. Please check with your doctor or pharmacist before pairing them.

What Is Dorzolamide and Timolol Ophthalmic Used For?

Brand name: Cosopt

Dorzolamide and timolol ophthalmic eye drops are used to reduce elevated intraocular pressure (pressure inside the eye) in people with open-angle glaucoma or ocular hypertension (higher-than-normal eye pressure without signs of glaucoma damage). It is specifically intended for patients who have not responded well enough to beta-blocker eye drops alone.

This combination is available as Cosopt, Cosopt PF, and Dorzolamide Hydrochloride and Timolol Maleate eye drops. Cosopt PF is the preservative-free formulation, which may be preferred for people sensitive to preservatives.
