Rauvolfia vomitoria contains a very small amount of the drug reserpine. Reserpine can reduce both systolic and diastolic blood pressure.

Here's the simple version: iloprost (Ventavis) already lowers blood pressure because it relaxes and widens your blood vessels, including the ones in your lungs. Rauvolfia vomitoria contains a small amount of reserpine, which also nudges blood pressure down. Stack the two together and you could end up with too much of a good thing, meaning your pressure drops lower than intended.

In real life that might show up as dizziness, lightheadedness, feeling faint when you stand up, or unusual tiredness. To be fair, this is a theoretical concern based on how these two work, not something proven in studies, so don't panic. Still, since iloprost is serious medicine for a serious condition, please run this combination by your pharmacist or doctor before mixing them.

Theoretically, combining Rauvolfia vomitoria with antiplatelet or anticoagulant drugs might have additive effects.
Rauvolfia vomitoria contains small amounts of yohimbine. Research in healthy adults shows that taking yohimbine in doses of 8 mg or more seems to inhibit platelet aggregation in vitro by binding to the alpha-2 adrenoceptor. The effects of Rauvolfia vomitoria itself are unclear.

Here's the gist for someone using Iloprost (Ventavis) for pulmonary hypertension. Iloprost doesn't just relax blood vessels in your lungs, it also makes your platelets a little less sticky, so it has a mild blood-thinning side to it. Rauvolfia vomitoria contains a small amount of yohimbine, and in lab studies yohimbine can also slow platelets down. So on paper, putting the two together might add up and make it a touch easier to bruise or bleed.

I'll be honest though, this concern is theoretical. It comes from test-tube work with isolated yohimbine, not real-world studies with the actual plant, and it's rated a minor interaction. The bigger reason to be cautious is that Rauvolfia strongly lowers blood pressure on its own, which is a real issue when you're already on a vasodilator. Please loop in your doctor or pharmacist before combining them.
