Metformin XR falls under ANTIDIABETES DRUGS — the group Rauvolfia Vomitoria interacts with.

Theoretically, taking Rauvolfia vomitoria with antidiabetes drugs might increase the risk of hypoglycemia.
Preliminary clinical research shows that drinking a beverage containing Rauvolfia vomitoria foliage and bitter orange fruit lowers blood glucose levels. Hypoglycemic effects of Rauvolfia vomitoria have also been demonstrated in an animal diabetic model.

Here's the simple version: Rauvolfia vomitoria may have a mild blood-sugar-lowering effect of its own. Metformin XR already works to bring your glucose down, so stacking the two together could, in theory, push your blood sugar a little lower than expected. That might show up as shakiness, sweating, dizziness, hunger, or feeling foggy.

I want to be honest about the evidence, though. This is mostly theoretical, based on a small beverage study and an animal model, not strong human trials. On its own, metformin rarely causes true low blood sugar. Still, since Rauvolfia is a potent herb that also affects blood pressure and mood, please check with your doctor or pharmacist before combining them, and keep an eye on your glucose readings if you do.
